- 5-Digit Zip – Enter the zip code of the city or town where your equipment will be.
- MC# -- Enter your MC # issued to you by the FMCSA. (Not required)
- Equipment Type – Select from the three available options in the drop-down box.
- Information on how we will send notifications (How To Contact)
- Form To Contact – Select the method you would like us to use to send notifications by clicking on the appropriate radio button.
- Name – Enter your name.
- Number – If you want to be notified by Text Message, enter the formatted 10-digit phone number for your cell phone.
- Email – If you want to be notified by Email, enter your complete email address.
